A report released by the Japan Transport Safety Board on Wednesday has concluded that human error was the primary cause of a fatal collision at Tokyo's Haneda Airport on January 2, 2024. The incident, which involved a Japan Airlines (JAL) passenger plane and a Japan Coast Guard aircraft, resulted in five deaths and significant damage.

The pilot and co-pilot of the Japan Coast Guard plane mistakenly believed they got cleared to enter the runway just before the Jan. 2 incident.

A report confirmed prior indications that a pilot’s misunderstanding of air traffic control directives, and a missed warning, caused the January accident.

A two-plane collision in Tokyo on Jan. 2 happened because a pilot mistakenly believed he had been given permission to enter the runway for takeoff, a crash investigation found.
